Join us for a comprehensive workshop designed to empower healthcare professionals with the tools to provide culturally safe, trauma-informed, and clinically effective care.

This intensive professional development workshop aims to enhance the capability of primary healthcare providers to deliver culturally safe, effective, and collaborative chronic disease management services for Aboriginal and Torres Strait Islander populations. The goal is to move beyond conventional clinical approaches by grounding practice in an understanding of historical context, social determinants, and the crucial role of community-led care, ultimately improving health outcomes and closing the gap in health equity.

LEARNING OUTCOMES:

  • Analyse the historical, social, and cultural determinants of health, and articulate how historical trauma and systemic barriers contribute to current health inequities.
  • Identify the high prevalence and specific risk factors for chronic conditions such as diabetes, cardiovascular, respiratory diseases within Aboriginal and Torres Strait Islander communities and integrate cultural considerations into diagnostic and treatment plans.
  • Apply the principles of cultural safety, cultural humility, and trauma-informed care to build respectful, trusting, and effective therapeutic relationships with your patients.
  • Demonstrate culturally safe communication strategies through practical exercises and role-playing scenarios.
  • Implement collaborative care models to optimize chronic disease management pathways.
  • Conduct a comprehensive 715 Health Assessment and effectively plan follow-up care, including appropriate allied health and community referrals, utilizing existing support networks and resources.
